Men considering penile enlargement in New York City often assume that increasing penile size requires surgery, implants, or a prolonged recovery. That is no longer always the case.

For appropriately selected men, non-surgical penile enlargement using hyaluronic acid (HA) filler can increase penile girth and improve contour without incisions, general anesthesia, or traditional penile enlargement surgery.

But penile enhancement is not simply a matter of “injecting filler.”

The quality of the result depends heavily on understanding penile anatomy, filler selection, injection plane, volume, symmetry, and three-dimensional penile contouring. The goal should not be an obviously augmented penis. The objective is a penis that is larger while maintaining a natural appearance, proportion, and feel.

As a urologist specializing in male sexual medicine, penile reconstruction, penile prosthetic surgery, and penile enhancement, I approach penile augmentation as a medical and anatomical procedure—not simply a cosmetic injection.

What Is Non-Surgical Penile Enlargement?

Non-surgical penile enlargement is an office-based procedure in which a biocompatible injectable material is strategically placed beneath the penile skin to increase circumference and improve the overall shape of the penis.

In my practice, the emphasis is primarily on hyaluronic acid penile filler.

Hyaluronic acid is a naturally occurring substance found throughout the human body and is widely used as a soft-tissue filler. Published clinical studies have demonstrated measurable increases in penile circumference following HA penile augmentation, although results and durability vary among patients.

Unlike permanent injectable materials, HA also has an important advantage: it can potentially be modified or dissolved with hyaluronidase when clinically appropriate.

Penile Enlargement vs. Penile Sculpting

I prefer to think of the procedure as penile sculpting rather than simply penile enlargement.

Why?

Because adding volume alone does not necessarily create an attractive or natural result.

The penis is a three-dimensional structure. The physician must consider:

  • Baseline penile circumference
  • Shaft length
  • Proximal versus distal girth
  • Symmetry
  • Existing irregularities
  • Skin mobility
  • Circumcision status
  • Glans-to-shaft proportions
  • The patient’s desired degree of enhancement

The objective is controlled augmentation that creates girth, balance, symmetry, and proportion.

More filler is not automatically better.

What Actually Happens During a Penile Filler Procedure?

1. Consultation and Anatomical Evaluation

The process begins with a private consultation.

I evaluate penile anatomy and discuss what the patient wants to change. Some men want a modest improvement. Others are seeking a more substantial increase in circumference.

This is also an opportunity to determine whether there are underlying conditions such as Peyronie’s disease, erectile dysfunction, previous penile surgery, penile implants, fibrosis, or other anatomical abnormalities that could affect treatment planning.

Just as importantly, expectations need to be realistic.

The objective is enhancement—not the creation of an anatomically unrealistic result.

2. Designing the Desired Shape

Before treatment, we determine the areas that require augmentation.

Penile filler should not simply be deposited uniformly without consideration of anatomy.

The treatment can be planned to address areas such as:

Overall shaft girth
Increasing circumference along the penile shaft.

Proximal girth
Creating greater volume toward the base when appropriate.

Distal contour
Maintaining a natural transition between the shaft and glans.

Asymmetry
Correcting areas that appear disproportionately narrow or irregular.

Penile sculpting
Creating a smoother and more balanced cylindrical appearance.

This individualized planning is one of the most important differences between penile sculpting and simply injecting filler.

3. Local Anesthesia

Non-surgical penile enhancement is generally performed using local anesthesia.

There is typically no general anesthesia and no surgical incision.

Once adequate anesthesia has been achieved, the filler can be introduced into the appropriate tissue plane.

4. Strategic Placement of Hyaluronic Acid

The HA filler is carefully distributed along the shaft.

Placement requires an understanding of penile fascial anatomy and the relationship between the skin, superficial tissues, neurovascular structures, and erectile bodies.

The objective is to create an even layer of augmentation while preserving normal penile function and mobility.

The filler is then carefully shaped to achieve the desired contour.

This is where experience with penile anatomy becomes particularly important.

The penis is not simply a larger version of an area typically treated with cosmetic filler. It is a dynamic sexual organ that changes dramatically between the flaccid and erect states.

How Much Larger Can the Penis Become?

Most non-surgical penile enlargement procedures are designed primarily to increase girth rather than erect penile length.

Published clinical trials of HA penile augmentation have demonstrated significant increases in penile circumference. One randomized study reported an average girth increase of approximately 2.3 cm at 24 weeks, while another trial demonstrated persistent augmentation at 48 weeks, although the magnitude of enhancement declined over time.

These numbers should not be interpreted as a guarantee.

The achievable change depends on the patient’s anatomy, starting dimensions, amount and characteristics of filler used, and desired result.

I prefer to establish a treatment plan based on proportion rather than pursuing a predetermined number.

Does Penile Filler Increase Length?

This is an important distinction.

HA penile filler is primarily a girth-enhancement procedure.

Some patients may perceive the penis as larger or more substantial in the flaccid state following treatment, but filler should not be represented as a reliable method for significantly increasing erect penile length.

Patients specifically concerned about penile length require a different evaluation because apparent shortening can have several causes, including suprapubic fat, penile retraction, Peyronie’s disease, fibrosis, previous surgery, or loss of erectile quality.

Will the Penis Feel Natural?

Maintaining a natural appearance and feel is one of the principal objectives of my technique.

Properly selected HA filler can integrate into the soft tissues while allowing the penis to remain mobile and functional.

However, outcomes depend on several factors, including the product used, quantity injected, tissue characteristics, injection technique, and post-procedure healing.

This is another reason I emphasize penile sculpting rather than simply maximizing volume.

Is Penile Filler Permanent?

No.

Hyaluronic acid is gradually broken down by the body.

The duration varies between individuals, and patients should expect some gradual reduction in volume over time.

Published studies have documented measurable enhancement at approximately 6–12 months, but long-term durability varies and additional research is needed.

Some men eventually choose a maintenance treatment or additional augmentation.

One advantage of HA over permanent materials is precisely that it is not intended to be permanent and can also potentially be treated with hyaluronidase if correction is necessary.

What Is Recovery Like?

One reason men choose non-surgical penile enlargement is the relatively limited downtime compared with surgery.

Patients should expect temporary swelling and possibly bruising, tenderness, or contour changes as the tissues settle.

Specific post-procedure instructions are provided regarding hygiene, activity, sexual activity, and when manipulation or massage is appropriate.

Sexual activity should not be resumed until the treated tissues have adequately healed and the physician has cleared the patient.

Is Non-Surgical Penile Enlargement Safe?

No penile enhancement procedure is completely risk-free.

Published studies of HA penile augmentation have generally reported favorable safety profiles, but complications can occur. Reported adverse events include swelling, bruising, pain, inflammation, nodules, asymmetry, contour irregularities, filler migration, infection, and other uncommon but potentially significant complications.

A 2026 systematic review found that HA had a comparatively favorable complication profile among the augmentation techniques evaluated, while emphasizing that the evidence remains heterogeneous and that better comparative studies are still needed.

This is why patient selection, sterile technique, anatomical knowledge, appropriate filler placement, and physician experience matter.

Why Have Penile Enhancement Performed by a Urologist?

Penile filler occupies a unique area between cosmetic medicine and reconstructive urology.

A physician performing the procedure should understand more than filler.

He or she should understand:

  • Penile vascular anatomy
  • Penile fascial planes
  • Erectile function
  • Peyronie’s disease
  • Penile fibrosis
  • Previous penile surgery
  • Penile prostheses
  • Penile deformity
  • Sexual function
  • Management of potential complications

The best penile enhancement should not immediately look like penile enhancement.

Increase size while preserving proportion, symmetry, natural appearance, natural feel, and sexual function.

That is the difference between simply adding volume and performing anatomically planned penile sculpting.
